We are deeply invested in our core value to put patients first, and also deeply committed to creating a culture where every employee is valued and we learn and improve together. </p> <p><strong>About the Role:</strong></p> <p>We are seeking Physicians specialized in <strong>Endocrinology and Obesity Medicine </strong>to join our innovative, multidisciplinary chronic disease management team<strong>, </strong>and preferably hold a LOQ (Letter of Qualification) with the IMLC (Interstate Medical Licensure Compact) if living in an IMLC eligible state. </p> <p>This new role is <strong>Modified FTE (32 or 36hrs/week) role, </strong>designed for an American Board of Internal Medicine (ABIM) certified <strong>Endocrinologist</strong> with demonstrated expertise in <strong>obesity medicine, comprehensive diabetes management</strong>, and a strong interest in working collaboratively with Endocrine Advanced Practice Providers (APPs) in providing high-quality patient care.</p> <p>A critical aspect of this role is ensuring the safe and effective management of patients with complex co-morbidities, including <strong>Chronic Kidney Disease (CKD), End-Stage Renal Disease (ESRD), and Cardiovascular conditions</strong>. The physician will help enhance the consistency, safety, and effectiveness of patient care across the team, specifically integrating the use of <strong>Continuous Glucose Monitors (CGMs)</strong> and <strong>insulin pump</strong> therapy.</p> <p>This role will include participation in a rotating after hours call schedule with Form Health clinical leadership and will have a reduced clinical schedule the weeks of on-call coverage.</p> <p>This is a full-time position based in a remote, telemedicine clinical setting. The physician will work closely with a supportive care team and have protected time for supervision, teaching, and administrative duties.</p> <h3><strong>What You Will Do:</strong></h3> <ul> <li>Provide direct patient care in a telemedicine clinical setting, focusing on evidence-based treatment for obesity and comprehensive diabetes management, including insulin titration and care of patients who may already be using insulin pumps and/or continuous glucose monitors (CGMs).</li> <li>You have your own panel of patients, and the role could also involve collaborating with APP colleagues and Registered Dietitians specializing in diabetes treatment, primarily Type 2 Diabetes. This role could involve APP collaboration and supervision. </li> <li>Review charts and conduct initial and return visits for APP patients every third visit to ensure quality, continuity of care, and appropriate management of CKD/ESRD and CAD/CV conditions.</li> <li>Provide advanced treatment planning and medical management support for complex endocrinology cases, escalated concerns, and patients requiring specialized management protocols.</li> <li>Prescribe non-controlled medications, including those for obesity and diabetes, for patients under both direct care and in coordination with APPs.</li> <li>Conduct weekly 30 minute collaborative meetings and 30 min asynchronous communication time with each assigned Endocrine-Specialized APP to review cases, address clinical challenges, and provide ongoing mentorship.</li> <li>Ensure clinical documentation, protocols, and treatment pathways are followed consistently within the team, with a focus on guidelines for managing complex diabetic populations.</li> <li>Assist in the professional development of colleagues by fostering clinical growth, offering feedback, and supporting continuing education in both endocrinology and obesity medicine.</li> <li>Participate in team-based care initiatives and contribute to a culture of collaboration and high-quality care.</li> <li>Attend relevant departmental meetings and contribute to quality improvement efforts within the diabetes and obesity medicine program.</li> </ul> <p><strong>About You:</strong></p> <ul> <li><strong>Required:</strong> Board certification in Endocrinology by the American Board of Internal Medicine (ABIM)</li> <li><strong>Highly Preferred:</strong> Certification by the American Board of Obesity Medicine (ABOM) or demonstrated extensive clinical experience (minimum 3 years) in obesity medicine and pharmacotherapy (older generation oral and newer injectable medications). </li> <li><strong>Required: </strong>Active and unrestricted Medical State License in any <strong>US State </strong>and<strong> preferably holding</strong> a LOQ (Letter of Qualification) with the IMLC (Interstate Medical Licensure Compact) if living in an IMLC eligible state.</li> <li>Minimum of 5 years as an attending physician in endocrinology.</li> <li><strong>Demonstrated competency and comfort in managing patients with complex co-morbidities, including CKD, ESRD, and CAD/CV conditions.</strong></li> <li><strong>Expertise in advanced diabetes management, including the use of CGMs, insulin pumps, and complex insulin regimens.</strong></li> <li>Strong interest in mentoring, teaching, and collaborating with Endocrine-Specialized APPs and <strong>Registered Dietitians specializing in diabetes</strong>.</li> <li>Demonstrated experience working with APPs, medical residents, or trainees in a clinical academic setting.</li> <li>Excellent communication and interpersonal skills.</li> <li>Adherence to evidence-based treatments, behavioral interventions, nutrition, and pharmacotherapy for both endocrine disorders and obesity.</li> <li>Committed to delivering patient-centered, culturally competent care.</li> <li>Bilingual candidates with medical fluency in Spanish are highly preferred</li> </ul> <p><strong>More about Form Health’s benefits:</strong></p> <ul> <li>Competitive salary</li> <li>Comprehensive health benefits</li> <li>401k program</li> <li>Flexible work schedules and paid time off</li> <li>Paid parental leave</li> <li>Ability to participate in Form4Form, Form Health's weight loss program available to all employees and their dependents.</li> </ul> <p><strong>Form Health’s commitment to building a diverse, equitable, and inclusive work environment:</strong></p> <p>Form Health is committed to creating a culture and environment that celebrates diversity and inclusion, while fostering safety and belonging.
