<p>Reports To: Director of Behavioral Health or Behavioral Health Manager</p><p>Section I: Job Purpose</p><p>Pacific Health is expanding its team and seeking a highly skilled, compassionate therapist for a hybrid role. This role involves conducting comprehensive assessments, developing individualized treatment plans, delivering evidence-based interventions, and maintaining accurate clinical documentation. The therapist will play a vital role in helping clients achieve their mental health goals and improve their overall well-being. The therapist is required to maintain the confidentiality of client information; write and interpret written clinical information accurately and effectively.</p><p></p><p>Section II: Key Responsibilities </p><p>Clinical Care &amp; Intervention</p><ul><li>Deliver direct and telehealth therapy services to clients via phone, video, and in-person.</li><li>Provide crisis intervention and short- and long-term psychotherapy.</li><li>Conduct mental health evaluations and formulate treatment plans.</li><li>Offer referrals and case management services.</li></ul><p>Documentation &amp; Compliance</p><ul><li>Maintain up-to-date, accurate clinical records in compliance with HIPAA.</li><li>Ensure all documentation is completed within established deadlines.</li><li>Adhere to mandated reporting protocols.</li></ul><p>Collaboration &amp; Communication</p><ul><li>Engage with case management, administrative, and leadership teams to ensure coordinated care.</li><li>Maintain professional, timely communication via email, phone, and instant messaging.</li><li>Participate in team meetings, trainings, and interdisciplinary planning.</li></ul><p>Work Ethic &amp; Professionalism</p><ul><li>Work independently with minimal supervision.</li><li>Demonstrate punctuality, reliability, and responsiveness.</li><li>Uphold a high standard of customer service and empathy.</li></ul><p></p><p>Section III: Knowledge &amp; Skills Education</p><ul><li>A Master's degree from an accredited college or university in psychology, marriage and family therapy, social work, or counseling. </li></ul><p>Licensure</p><ul><li>Must hold a current California license as one of the following:</li><li>Associate Marriage and Family Therapist (AMFT)</li><li>Associate Clinical Social Worker (ACSW)</li><li>Associate Professional Clinical Counselor (APCC)</li></ul><p>Experience</p><ul><li>Minimum 2 years of experience in treating mental health consumers.</li><li>Case management experience is preferred.</li></ul><p>Language Proficiency</p><ul><li>Fluent in written and spoken English.</li><li>Bilingual in Spanish and/or Arabic is preferred.</li></ul><p>Technical Skills</p><ul><li>Proficient in Google Suite (Docs, Sheets, Slides) and Gmail.</li><li>Strong written and verbal communication.</li><li>Analytical and multitasking abilities.</li></ul><p>Interpersonal Traits</p><ul><li>Compassionate and empathetic approach to patient care.</li><li>Collaborative spirit and professional demeanor.</li></ul><p></p><p>Section IV: Key Interactions Internal Contacts</p><ul><li>Patients – Direct care and communication.</li><li>Director of Behavioral Health, Chief Clinical Officer, Administrative Staff, Patient Coordinators – Collaboration and care coordination.</li></ul><p></p><p>Core Responsibilities:</p><ul><li>Establishes positive, trusting rapport with patients.</li><li>Interviews and conducts diagnostic assessments of patients to develop treatment and recovery plans for patients and/or their family members.</li><li>Participates in the intake process by interviewing patients and/or their family members to obtain family, psychosocial, employment, medical, and mental health treatment history.</li><li>Provides crisis intervention, treatment, case management, referrals, and other related mental health care services to patients and their family members.</li><li>Participates in the development of treatment plans designed to address mental, emotional, and family disorders and dysfunction.</li><li>Provides ongoing clinical assessment, treatment planning, and clinical treatment for patients and families.</li><li>Prepares and maintains clinical documentation, in addition to adhering to HIPAA regulations.</li><li>Completes administrative requirements including but not limited to timely completion of documentation by set deadlines.</li><li>Collaborative work with case management team, administrative team, and management team.</li><li>Participates in clinical supervision and maintains supervision log.</li><li>Prompt responsiveness over email, phone, instant messaging, punctuality, and professionalism.</li><li>Willingness to provide mental health services via telehealth to include phone and/or video calls while following mandated reporter protocols.</li><li>Must be able to demonstrate independent work with minimal supervision.</li><li>Collaborating with the Enhanced Care Management (ECM) team on comprehensive case conceptualizations to optimize member outcomes.</li></ul><p></p><p>Work Location: Remote in San Bernardino, CA</p><p>Job Type: Full-time; 10am - 7pm PST</p><p>Pay: $29.00 - $33.00 per hour</p> <p>Required:</p><ul><li>Must maintain a current license with the California Board of Behavioral Sciences as an Associate Marriage and Family Therapist (AMFT), Associate Clinical Social Worker (ACSW), OR Associate Professional Clinical Counselor (APCC)</li><li>Computer skills and proficiency in office suites (Google), Spreadsheets (Google Spreadsheets), operating system (Windows/ MAC), communication and collaboration (Gmail).</li><li>Strong analytical thinking and ability to handle multiple tasks concurrently.</li><li>Excellent customer service with patients &amp; co-workers.</li><li>Excellent communication and listening skills.</li><li>Ability to build rapport quickly.</li><li>Detail oriented.</li><li>Ability to communicate with compassion and empathy.</li></ul><p></p><p>Minimum Qualifications:</p><ul><li>A Master's degree from an accredited college or university in psychology, marriage and family therapy, social work, or counseling.</li><li>Possession of an active, current, and clear registration with the California Board of Registration with the Board of Behavioral Health Sciences (BBS) REQUIRED</li><li>Must be a mental health professional as an Associate Clinical Social Worker (ACSW), Associate Professional Clinical Counselor (APCC), Associate Marriage and Family Therapist (AMFT)</li></ul> <p>Benefits &amp;
