<p><span>SUMMARY</span></p> <p> </p> <p><span>Essential Duties and Responsibilities include the following. Other duties and tasks may be assigned.</span></p> <p> </p> <p><span>Primary duties include:</span></p> <ul> <li><span> ESKD rounding, including limited and comprehensive visits, under the guidance of practice nephrologists</span></li> <li><span> ESKD hollistic patient care management including but not limited to patient activation, depression, high risk of hospitalization, transition of care and co-morbid medical issue oversight</span></li> <li><span> ESKD patient education</span></li> <li><span> ESKD comprehensive disease management annual visits</span></li> <li><span> Care collaboration with with nephrologists, dialysis center IDT, Kidneylink IDT, hospital discharge care coordination team, PCPs and sub-specialisits as clinically indicated</span></li> <li><span> CKD clinical care and kidney disease education</span></li> <li><span> Other duties and tasks as assigned</span></li> </ul> <p><span>Functions performed during above duties:</span></p> <ul> <li><span> Perform physical examinations of patients to evaluate health and diagnose illnesses</span></li> <li><span> Order and assess diagnostic tests, such as x-rays and laboratory tests</span></li> <li><span> Determine, plan, and initiate appropriate treatment, and adjust treatment if necessary</span></li> <li><span> Prescribe medications</span></li> <li><span> Document medical information of patients and maintain electronic medical records</span></li> <li><span> Educate, instruct, and counsel patients and families on presenting health problems, treatment plans, and home care, and encourage and promote preventative health care and positive health behaviors</span></li> <li><span> Request consultation or referral with other health care providers, such as physicians and clinics, collaborate with Kidneylink RN Care Manager</span></li> <li><span> Actively promotes GUEST customer service standards; develops effective relationships at all levels of the organization.</span></li> <li><span> Participates in team concepts and promote a team effort; performs duties in accordance with company policies and procedures.</span></li> <li><span> Regular and reliable attendance is required for the job.</span></li> </ul> <strong><span>Qualifications</span></strong> <p><span>Qualifications/Requirements:</span></p> <p><span>To perform this job successfully, an individual must be able to perform each essential duty satisfactorily.</span></p> <p><span>Requirements include:</span></p> <ul> <li><span> Current license to practice as an Advanced Practitioner in the state performing duties</span></li> <li><span> Ideal candidate will have a firm understanding of the protocols, policies and operations of a physician practice.</span></li> <li><span> Must have necessary specialty certification 2 years clinical work experience Nephrology experience a strong plus.</span></li> <li><span>Excellent written and verbal communication skills</span></li> </ul>
