This is performed in accordance with physician orders and plan of care under the direction and supervision of the Director of Clinical Services/Clinical Supervisor or Manager.</div><div><br></div><div><strong>Why join our team:</strong></div><ul> <li>Treated with respect and dignity</li> <li>Ongoing training and development opportunities</li> <li>Supported in the field</li> <li>Flexible scheduling</li> <li>Travel time reimbursement</li> <li>Paid training</li> <li>Competitive Pay</li> </ul><div><br></div><div><strong>What you’ll be doing:</strong></div><ul> <li>Provide physician prescribed physical therapy.</li> <li>Improve or minimize residual physical disabilities of the patient.</li> <li>Return the individual to optimum and productive level within the patient’s capability.</li> <li>Participate with all other home care personnel in patient care planning.</li> <li>Documents all services provided, observations, client progress, and recommendations.</li> <li>Participate in interdisciplinary activities per Agency guidelines.</li> </ul><div><br></div><div><strong>What we’re looking for:</strong></div><ul> <li>A passion to serve and help others live their best lives possible.</li> <li>Graduate of an accredited physical therapy assistant program approved by the American Physical Therapy Association. </li> <li>Currently licensed in the state in which practicing</li> <li>Two (2) years’ experience preferred in a home health setting</li> <li>Knowledgeable and experience with Medicare OASIS documentation</li> <li>Licensed driver with automobile insured in accordance with state/Agency requirements.  Reliable transportation with good driving record.</li> <li>Preferably resides in Lorain County (Avon, Elyria, Lorain, Amherst, etc.)</li> </ul><div> </div> </div> <p>Flexible work from home options available.</p>
