We are currently seeking a <strong>Case Manager</strong> to join our <strong>Ohio Home Care</strong>&nbsp;program. As a <strong>Case Manager</strong>, you’ll work directly with individuals to assess their needs, develop personalized care plans, and connect them with essential services and supports. You’ll be part of a mission-driven team that values your expertise, supports your growth, and empowers you to make a real difference in your community.</p><p><strong>Key Responsibilities</strong><br></p><ul><li><p>Conduct face-to-face eligibility evaluations and assessments (e.g., Adult Needs and Strengths Assessment)</p></li><li><p>Must be available during business hours, 8:00 a.m. to 5:00 p.m., to complete all job duties, including but not limited to phone calls, home visits, and returning emails.</p></li><li><p>Travel to and from the individual’s home, not to exceed 2 hours each way.</p></li><li><p>Develop and maintain individualized, person-centered care plans and Health and Safety Action Plans, make required reports to regulatory bodies, and complete incident reports as needed.</p></li><li><p>Coordinate and authorize services across behavioral health, medical, and community-based systems</p></li><li><p>Transition planning, including coordinating care and services to support individuals during changes in providers, settings, or levels of care.</p></li><li><p>Assist individuals in accessing Medicaid and other essential services</p></li><li><p>Monitor service delivery and ensure compliance with program guidelines</p></li><li><p>Serve as the lead advocate and care coordinator for each participant</p></li><li><p>Adhere to the CareStar Rule in performance of job responsibilities.</p></li><li><p>Understand and comply with CareStar Policies and Procedures.</p></li><li><p>Maintain confidentiality as related to patient information.
