Responsibilities include assessing eligibility for placement, communicating with referral partners and guardians, and providing trauma-informed support, including de-escalation when needed. Requires multitasking across systems, navigating EHRs, and collaborating with internal and external teams to ensure timely, accurate outcomes. Ideal for candidates with strong communication, organization, and problem-solving skills who can work independently in a fast-paced environment. Minimum Requirements:Education: Bachelor’s degree in a relevant field (i.e. Social Work, Education, Sociology, Psychology, Counseling, Human Relations, or Criminal Justice) preferred. A high school diploma or equivalent with a minimum of two years of relevant experience (e.g., working with vulnerable populations, intake coordination, or crisis management may be considered.Licensure/Certification: Valid drivers license and auto insurance. At least 21 years of age. Experience: One or more years of related experience. Supervisory experience preferred. Preferred Experience/Skills: Preferred candidates will have at least two years working with vulnerable or at-risk populations, along with strong technical skills in Microsoft 365, cloud-based platforms, and Electronic Health Records systems. Candidates should be comfortable managing multiple digital tools simultaneously while demonstrating excellent multitasking, organization, and time management abilities. Strong verbal and written communication skills are essential, as is the ability to work independently, problem-solve, and collaborate effectively in a remote environment. Experience with crisis intervention, de-escalation techniques, and trauma-informed care is highly valued, along with a high level of attention to detail when handling intakes and patient data.
