<p>Description</p><p>The Physical Therapist will play a crucial role in providing high-quality rehabilitation services to our residents, with a focus on restoring mobility, improving strength and function, and enhancing overall quality of life.</p><p><br/></p><p><strong>Schedule: </strong>PRN, Weekdays, Weekends, 8 Hour Shifts - (<strong>We have Telehealth Options</strong>)</p><p><br/></p><p><br/></p><p><em><strong>Primary Responsibilities and Duties:</strong></em></p><ul><li>Evaluate Residents' physical abilities, functional status, and rehabilitation needs through comprehensive assessments.</li><li>Develop personalized treatment plans based on assessment findings, resident goals, and evidence-based practice guidelines.</li><li>Implement therapeutic interventions to address impairments, maximize functional independence, and promote safe mobility.</li><li>Provide one-on-one physical therapy sessions to residents, utilizing a variety of therapeutic techniques and modalities.</li><li>Monitor resident progress and adjust treatment plans as needed to optimize outcomes and achieve rehabilitation goals.</li><li>Collaborate with team members, including physicians, nurses, occupational therapists, and speech-language pathologists, to coordinate care and ensure continuity of services.</li><li>Educate residents and their families on exercise programs, safety techniques, and home exercise regimens to support ongoing rehabilitation and prevent re-injury.</li><li>Document all physical therapy services provided, including initial evaluations, progress notes, and discharge summaries, in accordance with facility policies and regulatory requirements.</li><li>Participate in resident care conferences, team meetings, and in-service training sessions to share expertise, contribute to care planning, and stay informed about best practices in rehabilitation.</li><li>Contribute to a positive and supportive work environment by demonstrating professionalism, teamwork, and effective communication skills.</li><li>Perform all other duties assigned.&nbsp;</li></ul><p>Requirements</p><p><em><strong>Job Qualifications:</strong></em></p><ul><li>Current state licensure or eligibility for licensure as a Physical Therapist.</li><li>Active CPR Certification, Fingerprint card, 2-step Tuberculin test</li><li>No previous experience in a skilled nursing facility or similar healthcare setting is required but preferred.</li><li>New Graduates are encouraged to apply.</li><li>Strong clinical reasoning and decision-making skills, with proficiency in evaluating and treating a diverse range of conditions and diagnoses.</li><li>Ability to build rapport with residents through a compassionate and caring demeanor.</li><li>Excellent interpersonal and communication skills, with the ability to collaborate effectively with residents, families, and team members.</li><li>Commitment to providing high-quality, resident-centered care with empathy and compassion.</li></ul><p><em><strong>Physical Requirements:</strong></em></p><ul><li>Work performed in a medical setting due to the need to work directly with residents.</li><li>Ability to stand, walk, and move around the facility for extended periods.</li><li>Frequent bending, stooping, reaching, and lifting of therapy equipment and assisting residents.</li><li>Manual dexterity and coordination for performing therapeutic exercises, manipulating therapy tools, and conducting assessments.</li><li>Strength and endurance to assist residents with mobility, transfers, and positioning as needed.</li><li>Visual acuity and auditory perception to observe and assess resident movements and communicate effectively with residents and team members.</li><li>Stamina and physical resilience to handle the demands of providing therapy services to multiple residents throughout the day.</li><li>Compliance with infection control protocols and safety precautions to minimize the risk of injury or illness to oneself and others.</li><li>Flexibility to adapt to various environmental conditions and accommodate the physical needs of residents with diverse mobility levels and functionality abilities.</li><li>Ability to operate therapy equipment and assistive devices safely and effectively, following proper training and protocols.</li><li>Maintain regular attendance and timeliness as set forth within the work schedule designated by the department supervisor/manager.</li><li>Job requires accepting criticism and dealing calmly and effectively with high-stress situations.</li><li>The ability to use your abdominal and lower back muscles to support part of the body repeatedly or continuously over time without "giving out" or fatiguing.</li></ul>
