<p>Are you a GP looking for meaningful, flexible clinical work — without the commute?</p><p>Proactive Care is seeking experienced General Practitioners for a fully remote telehealth position, delivering structured chronic disease management for veterans and patients with complex, multi-system health needs.</p><p>This is not a standard telehealth locum. You will provide clinical leadership within integrated Chronic Care Teams, working collaboratively with allied health professionals and specialists to translate clinical findings into coordinated, evidence-based care plans. The work is structured, purposeful, and genuinely multidisciplinary.</p><p>We work across both Medicare and DVA billing streams, with a dedicated focus on veteran health and chronic disease management. If you’re based in Sydney, the Central Coast, Newcastle, or anywhere across NSW with a reliable internet connection, this role offers the flexibility to work from home while doing clinical work that makes a real difference.</p>Remuneration<p>First 3 Months (Guaranteed) = $240.00 per hour</p><p>After 3 Months = 70% of billings</p><p></p><ul><li>Structured as an independent contractor arrangement</li></ul><ul><li>Bulk billing model across Medicare and DVA billing streams</li><li>Sign-on bonus and anniversary incentive available (details on enquiry)</li><li>Central Coast and Newcastle are DPA-eligible locations — GPs with a 19AB exemption, including international medical graduates, are welcome to apply</li></ul>Location<p>This is a fully remote telehealth position. There is no physical clinic attendance required.</p><p>We welcome applications from GPs based anywhere across NSW, including the Sydney metropolitan area, Central Coast, Newcastle, and regional NSW. All you need is a suitable home-office setup and a reliable internet connection.</p><p>GPs based on the Central Coast or in Newcastle may be eligible for additional DPA-related benefits, including pathways for those holding a 19AB exemption.</p>The Role<p>As a General Practitioner within our Chronic Care Team model, you will provide clinical leadership across structured telehealth case conferencing and patient review sessions:</p><ul><li>Lead and participate in scheduled telehealth case conferences as part of multidisciplinary Chronic Care Teams</li><li>Provide medical oversight and clinical direction for patients with complex chronic and multi-system conditions</li><li>Integrate findings from clinical assessments, pathology, imaging, and allied health input into coordinated, evidence-based care plans</li><li>Undertake chronic disease management (CDM) reviews and care planning, including GP Management Plans (GPCCMPs) and Team Care Arrangements (TCAs)</li><li>Collaborate with allied health professionals, specialists, and care coordinators to optimise patient outcomes</li><li>Deliver telehealth consultations for patient follow-up, clinical review, and ongoing management</li><li>Manage prescribing, referrals, and investigation requests as part of the coordinated care model</li><li>Ensure clinical safety, prioritisation, and escalation pathways are clearly identified and actioned</li></ul><p></p><p>Working Pattern: Case conferencing sessions are typically scheduled in structured blocks, with approximately 22 patient reviews conducted over an 8-hour session. Flexible scheduling is available, and sessions are conducted entirely via telehealth platforms.</p>Veteran Health & DVA Focus<p>Veteran Pathways & Proactive Care has a dedicated focus on veteran health and chronic disease management for both Medicare and DVA-funded patients.
