If you are passionate about making a positive impact in patients' lives and working in a supportive and family-oriented atmosphere, we encourage you to join us in providing exceptional home hospice care.</p><p>Please note that this role is per diem with a rate of $100 - $120 per visit.</p><p></p>Responsibilities<ul><li>Lead daily operations of our social services team, including policies, procedures, and care coordination with other clinical providers</li><li>Recruit, onboard, and mentor social workers—providing clinical supervision, observing practice, and supporting their professional growth</li><li>Oversee assessment, planning, and evaluation of social services care, and ensure clinical documentation meets state, federal, and ACHC standards</li><li>Partner with the Executive Director on the annual department budget and resource planning</li><li>Build strong knowledge of community resources and connect patients and referral sources to the right support</li><li>Plan in-service training and continuing education to keep the team’s skills sharp</li><li>Contribute to quality improvement initiatives and help identify new ways to better serve our patients</li><li>Perform other duties as required for patient services</li></ul> <ul><li>A Master of Social Work (MSW) from an accredited program</li><li>An active California license as an LCSW</li><li>At least 3 years of management experience, preferably in home care</li><li>Strong knowledge of healthcare regulations, ACHC standards, and business operations</li><li>Excellent communication skills and the ability to build trust with patients, families, staff, and the community</li><li>A self-directed, organized, and flexible approach—you take initiative and work well with others</li></ul> <ul><li>Flexible Schedule</li><li>Ability to Convert to Full-Time Based On Performance If Interested </li><li>Paid Sick Days</li><li>Paid Bereavement Leave</li><li>Skills Training & Development</li><li>Company Social Events</li><li>Mileage Reimbursement for Patient to Patient Visits</li><li>Referral Bonus</li></ul>
