As an Occupational Therapist with Best Home Health Providers, you will help patients regain independence, improve function, and safely navigate daily life — all from the comfort of their own home. </p><p>We value teamwork, empathy, and a commitment to high-quality service, all aimed at putting patients and their families at ease during their recovery. If you are passionate about making a positive impact in patients' lives and working in a supportive and family-oriented atmosphere, we encourage you to join us in providing exceptional home health care.</p><p>Please note that this role is per diem with a rate of $100 - $140 per visit.</p><p></p>Responsibilities<ul><li>Evaluate patients’ functional abilities and home environments to identify needs and safety concerns</li><li>Develop personalized treatment plans and communicate them to physicians and the care team</li><li>Teach and retrain patients in daily living skills affected by illness or injury</li><li>Recommend and instruct patients on adaptive equipment, splints, and prosthetic/orthotic devices</li><li>Maintain accurate clinical records and promptly report changes in patient condition</li><li>Coordinate with the broader care team to ensure seamless, patient-centered care</li><li>Educate family members and caregivers on the patient’s treatment approach</li><li>Supervise Occupational Therapy Assistants and oversee home health aide care when OT is the sole skilled service</li><li>Support discharge planning as patients transition out of home health care</li><li>Perform other duties as required for patient services</li></ul> <ul><li>Graduate of an occupational therapy curriculum accredited jointly by the Committee on Allied Health Education and Accreditation of the American Medical Association and the American Occupational Therapy Association</li><li>Registered with the National Registration Examination of the American Occupational Therapy Association</li><li>Active California Occupational Therapy license</li><li>Minimum 2 years of clinical OT experience</li><li>Current CPR certification with AHA</li><li>Valid driver’s license, reliable insured vehicle, and the ability to travel to patient homes</li><li>Strong communication and organizational skills</li></ul> <ul><li>Flexible Schedule</li><li>Ability to Convert to Full-Time Based On Performance If Interested </li><li>Paid Sick Days</li><li>Paid Bereavement Leave</li><li>Skills Training &amp; Development</li><li>Company Social Events</li><li>Mileage Reimbursement for Patient to Patient Visits</li><li>Referral Bonus</li></ul>
