In this impactful role, you'll play a critical part in coordinating patient care transitions and reducing preventable hospital readmissions. If you're passionate about improving patient outcomes and thrive in a fast-paced managed care environment, we want to hear from you!</div><div> <br><br> </div><div><strong>Responsibilities:</strong></div><ul> <li>Monitor and manage high-risk patients post-discharge to prevent unnecessary hospital readmissions</li> <li>Coordinate care transitions between hospitals, skilled nursing facilities, and outpatient providers</li> <li>Conduct telephonic and in-person patient assessments to identify barriers to care and social determinants of health</li> <li>Collaborate with interdisciplinary care teams including physicians, social workers, and utilization management staff</li> <li>Develop and implement individualized care plans focused on reducing readmission risk</li> <li>Track and document case management activities in electronic health record (EHR) systems</li> <li>Educate patients and caregivers on discharge instructions, medication adherence, and follow-up care</li> </ul><div><strong>Requirements:</strong></div><ul> <li>Active RN or Licensed Clinical Social Worker (LCSW) license in the state of California</li> <li>2+ years of case management, utilization management, or discharge planning experience</li> <li>Strong knowledge of managed care, IPA, or health plan operations preferred</li> <li>Experience with care transitions, readmission reduction programs, or chronic disease management</li> <li>Proficiency with EHR/case management software and Microsoft Office Suite</li> <li>Excellent communication, critical thinking, and organizational skills</li> <li>CCM (Certified Case Manager) certification is a plus</li> </ul><div><strong>About Us:</strong></div><div>MedPOINT Management is a leading Independent Practice Association (IPA) management company based in Sherman Oaks, CA, dedicated to delivering high-quality, coordinated healthcare to patients across Southern California.
