<p>Description</p><p> St. Croix Health is seeking a patient-focused, clinically driven <strong>Pharmacist</strong> to join our team in a full-time <strong>0.8 FTE</strong> position. The primary practice location is our <strong>Unity Clinic</strong> in Balsam Lake, WI, with the opportunity to provide coverage at our pharmacy locations in St. Croix Falls and Frederic as needed. Occassional remote work is available, offering additional flexibility while supporting our commitment to exceptional patient care. </p><p><br/></p><p>The Pharmacist is responsible for ensuring the safe and effective use of medications, providing high-quality pharmaceutical care, and promoting wellness to patients. The pharmacist works closely with other healthcare professionals to support optimal patient outcomes and contribute to the health and well-being of our community. </p><p><br/></p><p><strong>Prescription Preparedness</strong></p><ul><li>Dispense medications: Accurately prepare and dispense prescription medications according to legal, regulatory and organizational standards.</li><li>Patient consultation: Provide advice and information to patients regarding proper medication use, side effects, potential interactions and the importance of adherence to prescribed regimens.</li><li>Medication management: Review patient prescriptions and medication profiles to ensure safe and effective medication therapy. Identify and resolve potential medication-related problems</li><li>Medication error review: Monitor for medication errors and adverse drug reactions and document accordingly.</li><li>Compounding: Prepare customized medications for patients, when necessary, follow the specific instructions and requirements provided by prescribers and organization standards.</li><li>Collaboration with healthcare team: Work closely with physicians, nurses, and other healthcare providers to optimize patient care plans and medication therapy management. </li><li>Inventory management: Monitor and manage medication inventory to ensure adequate stock and proper storage conditions. Order medications and supplies as needed.</li><li>Patient education: Educate patients about managing chronic conditions, lifestyle changes, and preventative health strategies. Provide vaccination services where applicable.</li><li>Regulatory compliance: Ensure adherence to all relevant laws, regulations, and professional guidelines, including controlled substances laws.</li><li>Record keeping: Maintain accurate patient records, including prescriptions, medication therapy management services, and any other pertinent information.</li><li>Continuous education: Stay up to date with advancements in pharmaceutical science, treatment protocols, and industry best practices. </li></ul><p>* <em>While this job description is intended to be an accurate reflection of the job requirements, management reserves the right to modify, add or remove duties from particular jobs and to assign other duties as necessary.</em></p><p><br/></p><p><br/></p><p>Requirements</p><p><strong>Education & Licensure: </strong></p><ul><li>Graduate of a college of pharmacy accredited by the American Council of Pharmaceutical education</li><li>Current and valid Wisconsin Pharmacist license</li><li>Current BLS Certification</li><li>Immunization certification preferred within 6 months of hire </li></ul><p><strong>Experience:</strong></p><ul><li>Prior Pharmacist experience preferred</li></ul><p><strong>Knowledge, Skills & Abilities: </strong></p><ul><li>Excellent communication (verbal and written) and listening skills</li><li>Demonstrated ability to work independently with little direction and manage multiple demands and priorities </li><li>Able to handle frequent interruptions and adapt to changes in workload and work schedule</li><li>Able to set priorities, make critical decisions and respond quickly to requests</li><li>Fosters a team environment</li><li>Able to meet the pharmaceutical needs of all patient age groups</li><li>Self-directed and highly engaged, but also able to work well in a team</li><li>Ability to solve problems and encourage others in collaborative problem solving</li><li>Able to work cooperatively and effectively with providers, staff, and patients</li></ul><p><strong>Supervisory Responsibilities:</strong></p><ul><li>Maintain daily workflow of the department, giving directions to support staff</li></ul><p><strong>Physical Requirements</strong></p><ul><li>Prolonged periods of sitting at a desk and working on a computer</li><li>Prolonged periods of standing, bending, and reaching</li><li>Must be able to lift up to 50 pounds at times</li></ul><p> <strong>St. Croix Health is an Equal Opportunity Employer.</strong> <em><strong>We will ensure that individuals with disabilities are provided with reasonable accommodation throughout the hiring process.
