Your responsibilities include:</p><p>Clinical Leadership &amp; Oversight</p><ul><li>Providing overall medical direction to the program, including supervision of all hospice providers</li><li>Consulting with attending physicians on pain and symptom management for hospice patients</li><li>Acting as the primary medical resource to the hospice interdisciplinary group</li><li>Ensuring competent physician services are available on a 24-hour basis to meet the general medical needs of hospice patients</li><li>Reviewing and developing treatment protocols and proposing current, evidence-based intervention options</li><li>Maintaining current knowledge of the latest research and trends in hospice, palliative care, and pain/symptom management</li><li>Serving as collaborating physician for practitioners working within the organization, as needed</li></ul><p>Quality, Policy &amp; Compliance</p><ul><li>Establishing and continually reviewing policies and procedures related to patient care, medical education, and emergency protocols</li><li>Ensuring adherence to Medicare Conditions of Participation, CHAP standards, and applicable federal and state statutes</li><li>Evaluating and monitoring QAPI plans to identify medical education needs; participating in QAPI teams and activities</li><li>Developing criteria — in partnership with the COO and VP of Operations — to monitor the quality of physician and mid-level provider education programs</li><li>Assisting with evaluation of protocols and procedures with respect to quality and cost outcomes</li><li>Assuring that organizational policies, bylaws, rules, and regulations are followed throughout the program</li></ul><p>Education &amp; Community Engagement</p><ul><li>Acting as a liaison to physicians by providing consultation and education to IDG physicians, attending physicians, and community physicians related to hospice</li><li>Serving as a hospice champion — representing and promoting the program to physicians, discharge planners, referral sources, and community health organizations</li><li>Proposing and supporting organizational programs that address identified medical education needs</li><li>Demonstrating skill in counseling and communicating with patients and families navigating end-of-life care</li><li>Participating in resolution of interpersonal conflict and issues of clinical and ethical concern</li><li>Engaging in ongoing medical education activities related to hospice and palliative care</li></ul><p><strong>Benefits We Offer</strong></p><ul><li>Competitive pay</li><li>Company provided equipment</li></ul><p><br/></p><p><strong>Why Choose Transitions Hospice?</strong></p><p>Hospice physicians at <strong>Transitions </strong>aren't just consultants — they're leaders, educators, and advocates for a better end-of-life experience. Apply today.</p><p>Requirements</p><p><strong>What You Bring</strong></p><ul><li>Licensed as a Doctor of Medicine (MD) or Doctor of Osteopathy (DO) in the state without restriction or disciplinary action</li><li>Maintains controlled substances registrations with state and federal authorities</li><li>Not excluded from participation in the Medicare Program</li><li>Knowledge and prior experience in hospice, palliative care, and/or end-of-life care</li><li>Commitment to ongoing medical education in hospice and palliative care</li><li>Strong clinical judgment in pain and symptom management for terminally ill patients</li><li>Ability to serve as an effective educator, collaborator, and advocate across interdisciplinary teams and the broader physician community</li><li>Genuine commitment to the philosophy and values of hospice and end-of-life care</li></ul>
